Client was having numerous issues and incidents related to LOTO at their extraction plant. The plant itself was old, with zero-energy isolation impossible in certain systems. Additionally, as there was no standardized program for LOTO within the facility, many employees and contractors received varied training content, leading to inconsistencies.
